Hall of Fame Nomination Form

The Northwest High School Alumni Association is always accepting nominations for Hall of Fame. Nominations for the September 2018 Induction are due by May 15, 2018. Submit a nomination online or  download the nomination form here.

  • Distinguished Graduate: Must have graduated at least 20 years previously. Nominees will be considered for their outstanding success in his/her chosen profession and service to their community.
  • Faculty or Staff:  A teacher or staff member who is currently on staff will not be considered for any category until they leave NWHS or retire.
  • Outstanding NWHS Supporter:  This can be an individual or business that has supported the school in time or money in the past. Examples: Adopt-a-School partner, Past PTA Presidents, parents, alumni.
